CHDS is licensed by UDSMR to use their LIFEwareSM System assessment tool which has been designed and created by medical professionals. www.udsmr.org
Through the use of the LIFEwareSM System CHDS has the ability to:
- Cover a wide range of musculoskeletal, neurological, cardiopulmonary, cognitive and other medical conditions
- Offer detailed patient reports that help pinpoint specific treatment needs
- Provide data export capability which permits downloading into spreadsheets for further analysis
- Conduct On-line patient interviews
- Utilize Spanish language assessment forms
- Customize reports for analysis & quality improvement within one site or between multiple sites
- Help fulfill regulatory & accreditation requirements
The experience of LIFEwareSM System was developed by Uniform Data Systems with the goal of focusing on patient reported outcomes
- UDSMR arose from a University-based, NIDRR outcomes research study
- UDSMR wrote the book on rehabilitation & functional outcomes tracking
- LIFEwareSM System is based on a 2-year study of 7,000 rehabilitation assessments
- Current LIFEware® database holds over 500,000 assessments & 329,000 cases.
- UDSMR can produce reports across the Continuum of Care from inpatient to outpatient to home
- Total outcomes database available for research has over 6 million records
- Ongoing inpatient and outpatient research in all phases of outcomes tracking and reporting
CHDS offers the experience of Registered Nurses who are trained under the guidelines and criteria of UDSMR to ensure non biases outcomes and accurate results.
Upon request CHDS has the ability to use the LIFEware® assessment tool to customize forms with the measures that are used most frequently by your agency, giving the flexibility needed when reporting on a condition or problem.
